At Elm Hall, we care for people with a wide range of needs. Some of our residents live with a great degree of independence supported discreetly by staff with those little elements of day to day living that they find challenging. Others may need more intensive support which can include with daily care and activities of living. We can cater for most complex medical needs too such as catheter and stoma care, feeding tubes, palliative care and support for those living with dementia and alzheimers disease. Most important of all is the social care and activities provided to promote a good quality of life and to ensure every resident has a meaningful day filled with interraction and engagement. What We Offer
Nursing Care
Our team of NMBI Registered Nurses and QQI accredited Healthcare Assistants are skilled in providing compassionate individualised care for all your activities of daily living. Our team provide residents with discreet assistance with daily care such as showering, washing and dressing, mobilising, eating and drinking and medication management. We develop and constantly update your individual plan of care with you and your loved ones to make sure you get the help you need to live well.
Medical Care
At Elm Hall, our experienced General Practitioner (GP) Dr. Cliona Doyle co-ordinates and managed the healthcare needs of our residents supported by the nursing staff. Dr. Doyle holds clinic on site and provides emergency cover outside of hours through the Kildare and West Wicklow Doctor on call service (K-DOC). Many of our residents are cared for with on-site support from the Community Liaison Team from Connolly Hospital Blanchardstown led by Dr. Siobhan Kennelly, Consultant Geriatrician. We have close links with the Community Mental Health Team also.
Medicines Management
Medicines management is an important element of your care and we are proud to have partnered with Median Healthcare to provide a comprehensive medicines management service. The Clinical Pharmacist at Median is available to residents and their families to discuss medicines management and any queries they may have and provide education on best use of medicines. In conjunction with Dr. Doyle, the pharmacist and nursing staff review every residents medication on a quarterly basis or more often if required. Specific focus is given to ensuring medicines are used in a way that minimises risk of falls, reduces polypharmacy and respects residents rights.
Social Care and Activities
Social Care and meaningful activities are the key to our residents wellbeing. Our Activities Co-ordinators Margaret and Tommy prepare and co-ordinate our 7 day social and activities plan to provide activity, engagement, fun and community integration at Elm Hall. One to one activities and group activities are available. This is further supported by our network of trained, vetted volunteer visitors from Grandpal, and a network of visiting entertainers and social care providers such as Irish Therapy Dogs.
Allied Healthcare
We provide a full multidisciplinary care team service at Elm Hall which includes physiotherapy, podiatry, occupational therapy, dietician, speech and language, optometry and dental hygiene. Some services may be fully covered by the medical card, partially covered or subject to an additional fee.
